It's amazing what a monster Bull Rush can do to your party's bottleneck. Suddenly one monster pushes the Paladin back two squares, and then three more monsters rush the gap.

Just have your monsters make simple plans and it'll make a big difference.

Please, do not punish your characters for using tactical advantages. Simply have the monsters use them too. It is also wise to have them freak out rarely to one of those traps (rarely means rarely), especially since its funny.

I don't think a lieutenant would be angry with their soldiers for using a door as a good cover or bottleneck when outnumbered. They even used the almighty bottleneck in "the matrix" when those crappy flying spaghetti monsters were coming out of the ceiling of that city.

Doorway nonsense can be a little tiresome I suppose, but bottlenecking is a very legitimate tactic and so I wouldn't discourage it too heavily. Mostly it has the disadvantage of having the PCs stay in a tightly packed formation, making them good targets for area of effect spells and items (alchemist's fire and fireballs mostly). One of the best ways to stop an enemy from fireballing you is to stand real close (unless they're a devil, then do not do this).

I actually kind of like it when my players use cover during ranged combat

Start having the doors eat people. Have blade traps in the ceilings where the doors close. Have the ceiling of the room behind them slowly lowering. There's a dozen ways to pour the party out of one room and into another. You can do it, DM, we have faith in you.

More like D&D, however, is Doom’s enjoyably exploitable capacity for jamming monsters in narrow passageways in order to take them on one at a time.

My power-gaming player latched onto the "doorway principle" very early, and from then on ALL fights took place near or at any handy doorway. Plant the warrior and paladin at the door, and slaughter all who come through.
